Poorly maintained equipment and bad local management
Local management in Yukon was self protection mode. Made sure there was no competition for position. Was surrounded by incompetent and inexperienced personnel to ensure that. All crews knew that and morale was very low.
Yukon management was responsible for maintaining safe and quality equipment. That never happened. The equipment was in very bad shape. With all of the breakdowns cost this company some very good work.
Yukon management was also responsible for safety training of crews. New hire orientation was to take three days. Crews were put to work I half a day
