About
Description
As Care Coordinators, we are responsible for assessing client needs and monitoring the care provided to customers. We develop an adequate care plan and ensuring caregiver competency. We take part in the development of the highest possible standards of care. Providing guidance in all aspects of client care is also a part of our duty. We participate in monitoring and evaluating client services while upholding the company's standards. Working as Care Coordinators, we are responsible for delivering home care services, including client consultations. We respond to caregiver needs and concerns and advocate for them. Finally, we consult with supervisors and staff about special programs and services.
Core tasks:
- assuring that the level of care is safe for clients
- consulting with physicians relating to client care services
- directing the implementation of service goals and objectives
- maintaining standards that ensure safe and effective services
