About Care Transition Nurse

What does a Care Transition Nurse do?

Fulfilling the role of a Care Transition Nurse, we are responsible for identifying the right level of care the patient needs. We meet with physicians to discuss the well-being of patients. We must review high-risk patients. We educate on the benefit of home health care. We provide constant care through the entirety of the patient’s acute care recovery plan. As Care Transition Nurses, it falls to us to keep physicians updated on the status of patients. We work to prevent complications from arising. We educate family members on how to manage the health of the patient.

Core tasks:

  • collecting data on the patients for the doctor’s use
  • coordinating the processes of care of patients
  • supporting the patients, including family members during the care period
  • providing reports to the healthcare professionals in charge
