About Case Manager

What does a Case Manager do?

As case managers, we help people in managing their healthcare and social service requirements. We assist them in getting access to services. Our responsibilities include managing portfolios of cases and meeting with different clients regularly. Managing providers proactively and working with multiple clients to craft treatment plans are also our duties. Some of us work with individual service providers on a discharge plan. We accurately maintain and update cases.

Core tasks:

  • creating and maintaining effective relationships with important stakeholders
  • working as per health care best practices, relevant legislation, and internal policy and procedure
  • helping in managing patient health care requirements and services in areas like long-term care and mental health

As a case manager for 8 years, I see two issues. First, like an earlier commenter, the public often views direct service workers as people who love their calling and they see it as its own reward for these "special" dedicated soldiers. Donors then come to believe the workers don't need more than a pat on the back. The second reason is that direct service is nearly always a recipe for burnout. Employees can stave it off with adequate tools, training, and deep support, but those who last longer than a couple of years are a rare breed.
