About Clinical Care Manager

What does a Clinical Care Manager do?

As Clinical Care Managers, we hire, train, and evaluate care managers, RNs, and Coordinators. We maintain consistent communication with staff and conduct scheduled staff meetings. We provide clinical supervision to service line staff. We provide direct services within the care management services lines. We oversee the assessment and care plan development. We formulate care plans and service delivery for clients in conjunction with the team. We serve as a liaison between fiduciaries and other professionals to advocate for care services

Core tasks:

  • identify and measure care management outcomes
  • report client care, progress to the team on a continuous basis
  • oversee health care aspects of medication management, disease process, and self-care
  • coordinate in-home supports and Care Management Assistants
  • provide written personal care instructions
  • monitor communication logs
  • communicate regularly with the team regarding client progress
  • obtain necessary equipment and coordinate funding sources
  • attend physician's appointments to resolve medical issues for clients
