4 Star Nursing Home in Lawrenceburg, IN, seeks Full-Time Clinical Liaison Summary:

Shady Nook Nursing and Rehabilitation Center is hiring a Clinical Liaison to grow census by developing relationships with case managers, social workers and others in hospitals, nursing homes, and assisted living facilities in the Lawrenceburg, IN community and greater Cincinnati market. The Liaison also conducts clinical and financial screenings prior to admission and manages patient referrals to ensure a smooth transition into the nursing home.

Qualifications:

• RN/LPN/LSW, or 2+ years' experience in nursing home industry in one of the following roles.
• Clinical Liaison
• Director of Marketing
• Director of Admissions
• Central Admissions
• Experience with Medicare, Medicaid, and managed care insurances
• Experience reviewing referral information ensuring clinical, equipment, and medication needs can be met

Responsibilities:

• External marketing to hospital discharge planners, social workers and other referral sources to generate referrals/admissions and meet census goals
• Community outreach to healthcare professionals and community groups
• Clinical assessment of electronic medical charts from hospitals to determine if the facility can meet patient's clinical needs
• Communicate clinical information to facility
• Verification of benefits
• Obtain pre-authorizations from insurance company
• Ensure closure with each referral
