About Clinical Pharmacist

What does a Clinical Pharmacist do?

As Clinical Pharmacists, we are responsible for developing clinical pharmacy programs according to policies and regulations and reviewing records of patients to determine the appropriateness of medication therapy. We also have to evaluate the patient’s condition to ensure all issues are being treated. We have to collaborate with healthcare professionals to ensure optimal patient care. We need to keep accurate documentation of medication plans and patient progress.

  • reviewing records of patients to determine the appropriateness of medication therapy
  • evaluating patient’s condition to ensure all issues are being treated
  • identifying untreated health problems and refer patients to appropriate physicians
