About Director OF Care Management

What does a Director OF Care Management do?

As Directors of Care Management, we report to the System Director of Care Management. We assist in the creation and delivery of strategies focused on improving operations. We participate in the evaluation, planning, and execution of the full scope of Case Management functions. We help support and maintain consistency with system-wide initiatives. We translate organizational goals into action plans. We implement initiatives to meet strategic expectations.

Core tasks:

  • improve clinical and financial outcomes across the care continuum
  • develop and put in place best practices for Case Management teams and programs
  • meet strategic and operational excellence goals including cost initiative
  • oversee the execution of policies and procedures
  • assist teams in the evaluation and implementation of technology
  • assist in leading the health system Care Management Council and Care Management Operations Council
  • provide leadership support for the members of the group and committees
  • direct supervision of case management and utilization review functions
