About Director OF Case Management

What does a Director OF Case Management do?

As directors of case management, we supervise the staff of case managers who are responsible for patient care coordination. We design and implement different programs of case management including discharge or intake planning. Our responsibilities also include implementing managed care negotiation and contracting. Some of us evaluate data related to patient care to make sure proper care is given as per clinical guidelines. As directors of case management, we also contribute to the improvement and development of clinical care pathways that balance quality care and cost-effectiveness.

Core tasks:

  • accomplishing clients’ care by evaluating and monitoring treatment plans and assessing treatment needs
  • admitting new clients by reviewing applications and records and conducting orientations
  • establishing treatment programs by coordinating services being offered and setting routines and schedules
