Job Description

Our client is currently seeking a Director of Case Management

This job will have the following responsibilities:
  • Responsible for planning, organizing, coordinating and directing all functions of the department in order to meet the stated mission, vision and values.
  • It is expected that the Director will effectively operate the department, continue the improvement of the department's processes and outcomes, comply with hospital and corporate policy and procedures, efficiently manage finances and resources, promote teamwork and customer satisfaction, and integrate the department services into the organization's overall plan for the delivery of care and service.
Qualifications & Requirements:
  • Bachelor?s Degree Licensure/Certification/Registration Texas State RN license or compact license is accepted?
  • ?Experience 10 years nursing, 5 years management and 3 - 5 years of case management experience in an inpatient acute care hospital setting?
  • ?Strong care coordination and discharge planning experience is required.?


TASKS

  • Prepare and maintain records and case files, including documentation such as clients' personal and eligibility information, services provided, narratives of client contacts, or relevant correspondence.
  • Develop rehabilitation plans that fit clients' aptitudes, education levels, physical abilities, and career goals.
  • Monitor and record clients' progress to ensure that goals and objectives are met.
  • Confer with clients to discuss their options and goals so that rehabilitation programs and plans for accessing needed services can be developed.

KNOWLEDGE

  • Administration and ManagementKnowledge of business and management principles involved in strategic planning, resource allocation, human resources modeling, leadership technique, production methods, and coordination of people and resources.
  • ClericalKnowledge of administrative and clerical procedures and systems such as word processing, managing files and records, stenography and transcription, designing forms, and other office procedures and terminology.
  • Economics and AccountingKnowledge of economic and accounting principles and practices, the financial markets, banking and the analysis and reporting of financial data.
  • Sales and MarketingKnowledge of principles and methods for showing, promoting, and selling products or services. This includes marketing strategy and tactics, product demonstration, sales techniques, and sales control systems.
