About Field Case Manager

What does a Field Case Manager do?

As Field Case Managers, we are responsible for providing a complete and detailed continuum of unique care in accordance with the company’s standards and protocols by ensuring close communication between all members. We schedule training sessions to educate the staff on how to use the company’s software and how to analyze the database to facilitate their day-to-day activities while ensuring that they are briefed about any new law or regulation. We also assist and coach new members on how to perform a quick medication review and determine the overall health of the individual based on the doctor’s notes.

  • schedule visits to the patient’s houses
  • visit and assist clinics, hospitals, and other care institutions
