About Home Care Physical Therapist

What does a Home Care Physical Therapist do?

As Home Care Physical Therapists, we are responsible for providing health care services and follow-up at home. We evaluate the patient's condition by conducting examinations and assessments. We also document patient's health care data accurately. Scheduling and paying regular visits to patients is also part of our duty. We develop treatment plans as per patient conditions and health issues. We communicate these plans to physicians, staff, and patient's concerned persons. Working as Home Care Physical Therapists, we are responsible for coordinating with the patient care coordinator to make referrals to appropriate disciplines. Finally, we also conduct patient and disease-specific researches.

Core tasks:

  • educating people about healthcare services
  • monitoring and providing follow-up services to the patient at their homes
  • building and communicating patients healthcare treatment programs
  • submitting patient visit schedules to schedulers
