We also document patient's health care data accurately. Scheduling and paying regular visits to patients is also part of our duty. We develop treatment plans as per patient conditions and health issues. We communicate these plans to physicians, staff, and patient's concerned persons. Working as Home Care Physical Therapists, we are responsible for coordinating with the patient care coordinator to make referrals to appropriate disciplines. Finally, we also conduct patient and disease-specific researches.
