Job Description

Role_Overview:
CliftonLarsonAllen (CLA) Search has been retained by a mid-size senior living
organization, Tacoma Lutheran, to identify a seasoned MDS Coordinator to serv
their Executive Team by overseeing the daily operations of the Health Care
continuum for their community.

What_You ll_Do:
The MDS Coordinator oversees the daily operations of Skilled Nursing
environment. The MDS Coordinator will ensure compliance with the
organization s mission and values as well as federal, state, and local law
and regulations through a continuous quality improvement approach.

Responsible for the coordination of the admission, quarterly an
annual total care conferences for all R
Insures the accuracy, completeness and timeliness of the Minimu
Data Set (MDS) for all
Residents of the Care Center.
Assists the Primary Care Nurses (PCNs) in the completion of the
quarterly and annual
reports and facilitates the timeliness of these reports in reachi
the medical record.
Assists with determination of Resident eligibility for Medicare
coverage and facilitates
compliance with Medicare requirements for documentation and conti
Resident
certification.
Facilitates coordination of care between the disciplines of Nur
Rehabilitation
Services, Social Services and other disciplines as necessary to
insure that Residents are
receiving coordinated, comprehensive and continuous care as
determined in the Total
Care Conference.
In cooperation with Social Services facilitates the admission o
residents to insure a
smooth transition into the Care Center. This includes monitoring
admission process
to insure the completeness and accuracy of al admission document
Complete RAI process on all assigned residents according to OBR
PPS guidelines on
a timely basis.
Attend Patient Care Conferences on assigned residents.
Complete recapitulations of physician s orders.
Ensure consultant s recommendations are implemented.
Review Quality Indicator reports and assist with corrections as
needed.
Ensure that infection control logs are completed and submitted
ICN weekly.
Assist Unit Managers as needed.
Work with MRC to schedule RAIs for assigned residents.

What_You ll_Need:
Graduate of an accredited school of nursing
Currently licensed as a Registered or Licensed Practical Nurse
the State of Washington
Must have at least two years acute or long term care experience
Strong understanding of Medicare reimbursement and rehabilitati
nursing concepts
