About MDS Coordinator

What does a MDS Coordinator do?

As MDS Coordinators, we are responsible for identifying the needs of nursing home residents. We gather information on all patients. We carry out assessments of charts to review the welfare of residents. Crafting individualized health care plans for residents is one of our numerous duties. We assess the various needs of the patients. We work with the staff to make sure that all care plans are followed accurately for all residents. MDS Coordinators ensure that the hospital complies with all ethical standards. We work with health care teams to efficiently implement the health care plans for every resident.

Core tasks:

  • participating in the admission processes of residents
  • searching for potential risks to the Medicare reimbursement of the facility
  • ensuring that all admission process records are accurately documented
  • coordinating Medicaid and Medicare processes for qualified residents
