About Patient Care Coordinator

What does a Patient Care Coordinator do?

As Patient Care Coordinators, we are responsible for maintaining patient information and coordinating with caregivers and insurance companies. We handle all requests and inquiries for patient information and disperse this information according to federal and state regulations. Ensuring compliance with the medical facility’s policies and procedures and developing interdisciplinary care plans for patients is part of our main responsibilities. We orient and educate patients and their families and resolve issues that could affect smooth care progression. Working as Patient Care Coordinators, we foster peer support and monitor the delivery of care by completing patient rounds.

Core tasks:

  • report personnel and performance issues to unit manager
  • maintain communication with patients and staff
  • determine the effectiveness of patient care plans
