We handle all requests and inquiries for patient information and disperse this information according to federal and state regulations. Ensuring compliance with the medical facility’s policies and procedures and developing interdisciplinary care plans for patients is one of our main responsibilities. We orient and educate patients and their families and resolve issues that could affect smooth care progression. Working as Patient Information Coordinators, we foster peer support and monitor the delivery of care by completing patient rounds.
