What does a Registered Nurse Care Coordinator do?

As registered nurse care coordinators, we assist the patients by communicating vital information about health and wellness. We team up with other healthcare professionals to provide essential therapy to all patients in need. Often, we create healthcare plans projected towards the timely recovery of our patient. We monitor and evaluate the responses of each patient to treatment. We strive to communicate information necessary to follow up procedure to both Doctors and junior Nurses.

Core tasks:

  • create health goals for patients and monitor them
  • be the communication bridge between the care team, patients and their family
  • recruit and train care team members and organising schedules
  • ensure care is of the best quality and utmost confidentiality
  • manage patient's care and educate them regularly
  • create healthcare programs for each patient and supervise it
