About Registered Nurse Care Manager

What does a Registered Nurse Care Manager do?

As Registered Nurse Care Managers, we work directly with patients suffering from chronic illnesses and disabilities. We handle the development of patients' care plans. We also manage medications and research various treatment options. Being Registered Nurse Care Managers makes us an integral part of the primary care team. We ensure optimal care for vulnerable patients. We make sure that all standard precautions and sanitary precautions are in place daily. Supervising the patients’ follow-up treatment especially in the emergency department is one among our numerous duties. We also train patients’ family and junior staff members in the coordination of care needed for fast recovery and prevention of recurring disease. Registered Nurse Care Managers are burdened with the responsibility to ensure a smooth working relationship between all nurses, specialists and other health care providers.

Core tasks:

  • integrating the patients and their families into the necessary care management
  • informing and supporting patients and their families through difficult decision making
  • communicating patients’ condition with insurance companies and other healthcare specialists
  • updating patient’s healthcare plan according to progression and responses to therapy
