About Registered Nurse Case Manager

What does a Registered Nurse Case Manager do?

As Registered Nurse Case Managers, we are responsible for the creation and implementation of patient health care plans. We coordinate and effectively schedule patient care services. We act as liaisons between the patients, their doctors, and family members. It is within our duties to keep patient health care plans up-to-date and revised constantly. Patients and families are to be educated on the available health care options and plans by Registered Nurse Case Managers. We determine the patient’s eligibility for insurance in the case of specific treatments. We adjust treatment protocols as required and ensure they have sufficient finances to pay for the treatment.

Core tasks:

  • suggesting and executing necessary follow-up services
  • carrying out intake assessments with the patients and their family members
  • review the results from medical tests conducted
  • accurate documentation and submission of medical records
