Job Description

Full Time:  8:30 am - 5:00 pm 

Contract: 3 months- possible ext. 

Remote:  100%

RN License: Unrestricted Massachusetts RN Licensure

CERT:  CCM highly preferred: 

KEY RESPONSIBILITIES/ESSENTIAL FUNCTIONS* (in order of importance)

  • Through the use of clinical tools and review of member specific health information/data, conducts comprehensive assessments of referred members' needs and, in collaboration with the members' care team, determines an approach to resolve member issues and/or meet needs by evaluating the members' needs and available internal and external programs, services and resources

  • Conducts assessments in the most appropriate member environment, which may require travel to home, hospital and ambulatory care settings
  • Applies clinical judgment to the incorporation of strategies designed to reduce risk factors and barriers and address complex clinical indicators which impact care planning and resolution of member issues
  • Using advanced clinical skills, performs crisis intervention with members experiencing behavioral health or medical crisis and refers them to the appropriate clinical and service providers for thorough assessment and treatment, as clinically indicated. Provides crisis follow up to members to help ensure they are receiving the appropriate treatment/services
  • Effectively completes comprehensive assessments and the Minimum Data Set – Home Care (MDS-HC) in accordance with established workflows
  • Collaborates with Long Term Services Coordinators (LTS-C) in the completion of assessments for members with LTSS needs
  • Develops initial list of problems, goals and interventions based on assessment outcomes
  • Educates members on their integrated care team, resources available to them in Unify, and the benefits of engaging in collaborative care planning
  • Responsible for the completion of warm hand-off to care team lead for care plan completion in collaboration with the member and other members of the care team
  • Responsible for ensuring completion of medication reconciliation and management in collaboration with the care team 
  • Participates in risk management activities, including identifying and communicating issues of risk in a timely manner

  • Work in close collaboration with the ICT , including engages in care team huddles and during case reviews
  • Attend activities including clinical and other professional organization events as needed
  • Complies with established metrics for performance, adheres to documentation and work flow standards
  • Performs other duties as assigned
  • Maintain professional growth and development through self-directed learning activities and involvement in professional, civic, and community organizations

EDUCATION: (Minimum education & certifications required)

  • Baccalaureate Degree in Nursing
  • Current, unrestricted Massachusetts RN licensure
  • CCM preferred

EXPERIENCE:  (Years of experience)

  • Minimum 3 years of relevant experience
  • Extensive knowledge of Medicare and Medicaid, benefits and community resources
  • Strong knowledge of long-term care, behavioral health and chronic illness
  • Proven experience in completing in-home assessments including the use of the MDS-HC
  • Proven experience in care planning for complex members

SKILL REQUIREMENTS: (Include interpersonal skills)

  • Skill and proficiency in applying highly technical principles, concepts, and techniques that are central to the nursing profession
  • Skill in assessing, planning, and managing patient care as acquired through three years of clinical nursing experience in an acute care hospital; rehabilitation or home care experience
  • Advanced communication and interpersonal skills with all levels of internal and external customers
  • Knowledge of computer software applications; CCMS a plus
  • Good organizational and prioritizing skills 
  • Valid Driver's License and vehicle in good working condition, with ability and willingness to travel up to 10 times per month, in assigned geographic region and throughout the state as necessary
