About Colorado Blood Cancer Institute (CBCI): Blood Cancer and Stem Cell Transplant Experts Welcome to the Colorado Blood Cancer Institute(CBCI) , a part of the Sarah Cannon Cancer Institute at Presbyterian/St. Luke sMedical Center. Our specially trained hematologists are committed to advancingscience and care for patients with blood cancers such as leukemia, lymphoma andmyeloma. Having performed more than 3,000 transplants, Colorado Blood CancerInstitute is the largest and most experienced full-service blood and marrow(stem cell) transplant program in Colorado and among the top programs in thecountry. CBCI is accredited by the Foundation for Accreditation of CellularTherapies (FACT), the global standard for top quality patient care in cellulartherapies. We work closely with our physician colleagues from New Mexico toMontana to provide patients with nationally recognized care and access to themost innovative clinical trials. Position Summary: This position supports themission of Presbyterian/St Luke s Medical Center of optimizing quality of lifeof all those served by expanding beyond the traditional focus on medical needsto the needs of the whole person. Withinscope, the nurse navigator oversees and directs the care of the BMT orHematology-Oncology patient and is responsible for collaboration with alldisciplines to provide well-coordinated patient care. This position functionsto navigate blood cancer patients through the continuum of care includingdiagnosis, treatment, follow-up and long-term care needs. This includescoordination of chemotherapy orders, prescriptions and individualizedfollow-up. The hematology nurse coordinator demonstrates competence in the ability to triage patients over the phoneand in person. The hematology nursecoordinator demonstrates the ability to interpret laboratory and radiologictest results and communicates results to physician as necessary. They provide patient and family educationrelative to the patient s diagnosis and treatment process. The hematology nurse coordinator demonstratescollaboration, professionalism, and effective communication in all interactionswith physicians, the administrative team, PSLMC employees, patients, caregiversand referral sources. The hematologynurse coordinator acts as an ambassador for the bone marrow transplant programwhen communicating with other healthcare professionals, payors, and thecommunity. When the need arises,will perform other duties as assigned by Lead. OCN or BMT-CN certification within 18 months of hire. Education: Graduate of an accreditedschool of nursing, BSN preferred. Experience: Three to five yearsexperience in hematology-oncology or bone marrow transplant carepreferred. Special Qualifications: Ability to communicate effectively with patients, families, physicians,and other health care team members. Ability to manage a team and work with others within a team to ensurequality patient care. Strong critical thinking skills. Desire to provide compassionate, holistic care. Able to workindependently without close supervision.
