Withinscope, the nurse navigator oversees and directs the care of the BMT orHematology-Oncology patient and is responsible for collaboration with alldisciplines to provide well-coordinated patient care. This position functionsto navigate blood cancer patients through the continuum of care includingdiagnosis, treatment, follow-up and long-term care needs. This includescoordination of chemotherapy orders, prescriptions and individualizedfollow-up. The hematology nurse coordinator demonstrates competence in the ability to triage patients over the phoneand in person. The hematology nursecoordinator demonstrates the ability to interpret laboratory and radiologictest results and communicates results to physician as necessary. They provide patient and family educationrelative to the patient s diagnosis and treatment process. The hematology nurse coordinator demonstratescollaboration, professionalism, and effective communication in all interactionswith physicians, the administrative team, PSLMC employees, patients, caregiversand referral sources. The hematologynurse coordinator acts as an ambassador for the bone marrow transplant programwhen communicating with other healthcare professionals, payors, and thecommunity.
