Job Description
Interim HealthCare recognizes that our success begins and ends with all of our employees?both field staff and office staff! We are looking for a dependable Hospice Registered Nurse (RN) Case Manager who will manage the care of a defined caseload of patients to facilitate the healthcare team?s efficient and effective achievement of patient outcomes in accordance with the patient?s plan of care.
Responsibilities of the Hospice Registered Nurse (RN) Case Manager role include:Complete initial and ongoing comprehensive assessments of the patient?s needs.
Provide those services and/or treatments requiring substantial and specialized nursing skill.
Initiate appropriate preventative and rehabilitative nursing procedures.
Supervise paraprofessional and licensed professional team members.
Perform assessments and complete appropriate documentation.
Update plan of cares as necessary.
Registered Nurse (RN) with active license in Colorado.
Health care CPR/BLS certified.
Hospice or Home Health experience preferred.
Minimum one (1) year recent RN hands on experience.
On-Call rotation nights and weekends.
Excellent assessment and communication skills.
Able to work independently with minimal supervision.
