About Transition Coach

What does a Transition Coach do?

As Transition Coaches, we assist the director for special education in developing and monitoring a district-wide plan to provide effective transition services. We conduct informal and formal transition assessments and monitor the administration of transition assessments administered by campus staff with identifying strengths, needs, preferences, and interests. We are responsible for providing professional development and technical assistance for the transition service requirements and the appropriate implementation by campus staff regarding the transition opportunities for students served by special ED.

Core tasks:

  • provide information and coaching for students regarding transition and self-determination
  • work closely with campus staff and district administrators to provide parent coaching to support the transition for their child
  • conduct regular evaluations of transition services effectiveness and compliance
