About Transition Specialist

What does a Transition Specialist do?

As Transition Specialists, we are responsible for assisting students in their transition from one level to another. We build a cooperative link between school, home, and community to remove obstacles that impede student growth. We assist in developing strategies to ensure that every student is a productive member of society. Designing transition planning lessons is also part of our duty. We schedule and organize the class presentations for guiding students about re-entry and transition. Working as Transition Specialists, we collaborate on issues about student's educational plans. We conduct vocational assessment tests and interviews with students to know about their career interests. Finally, we develop professional growth plans depending upon the results of assessment tests.

Core tasks:

  • establishing links between school, home, and community
  • removing barriers that obstruct students growth
  • designing transition planning lessons
  • guiding students about transition and re-entry
