Responsibilities

The Director Population Health will work directly with the leadership of Adventist HealthCare Population Health and Post-Acute Care Services Division in the design and implementation of strategies for population health programs aimed to improve health outcomes, reduce health care spend, and improve quality and patient satisfaction. These programs will include a focus on post-acute care utilization, reduction of unnecessary hospital utilization, prevention of readmissions, and chronic disease management in the ambulatory practice setting. The Director Population Health will also work with Adventist HealthCare LifeWork Strategies in the design of employer based programs and with the Adventist Center for Health and Wellness and community partners in the implementation of programs aimed at social determinants of health. The Director will partner with other community partners including Nexus Montgomery, skilled nursing facilities, assisted living programs and other organizations within the community who are aligned with Adventist HealthCare to help meet its goals.

The Director will work closely with the IT departments of the respective organizations in the management of vendors and vendor analytics critical to the delivery of population health programs. The Director also is responsible for collaborating with the analytic team and the clinically integrated network leadership to determine necessary programs to meet contractual goals as well as to evaluate effectiveness of all population health programs on an ongoing basis. The role will lead financial and quality ROI analysis of Division programs.

The role directly supports AHCs Mission of extending Gods care by delivering population health management services, integrating with physicians, and improving the overall quality of health care delivered in the communities we serve. The Director supports population health efforts in coordination with appropriate stakeholders including:

Qualifications

Masters Degree

Strong leadership skills

Experience working with cross-functional teams in complex organizations on complex programs

Excellent time management and prioritization skills to focus on multiple projects simultaneously, adapt to change and work independently

Excellent oral and written communication skills. Ability to communicate effectively with members of healthcare team

Experience with computer systems required, including web-based applications and some Microsoft Office applications
