Health Promotion Consultant – Medicare

Location: Florida, New York, and Connecticut

Positions are able to work remotely; must reside within the state of Florida, New Jersey, or Connecticut

The Health Promotion Consultant is responsible for member outreach and engagement with emphasis on health education, quality of care improvement, closing gaps of care, removing barriers to care, and overall member satisfaction.

Primary duties may include, but are not limited to:

  • Makes telephonic outreach to members to inform of available quality programs, healthcare needs, and in person events.

  • Participates on in person health and wellness events.

  • Establishes positive relationships with members to gain understanding of their needs.

  • Recommends health education and quality improvement programs based on experience and feedback received from assigned member population.

  • Collaborates with key stakeholders to leverage available resources for development of new and accessibility to existing health education/promotion programs.

Qualifications

Required Qualifications:

  • Requires a BA; 2 years of experience; or any combination of education and experience, which would provide an equivalent background.

Preferred Qualifications:

  • Medicare experience highly desired.
