Job Description

Job Summary:

  • Supervise staff in the Care Management Services department, acting as a resource and role model for staff.
  • Ensure quality, cost-effective care for the identified patient population while adhering to Hospitals and departmental policies and procedures.
  • Assess patient's clinical, psychosocial status and current treatment plans and develop a comprehensive multidisciplinary plan of care utilizing tools and resources.
  • Collaborate with other departments/services and community healthcare agencies and act as a resource on complex patient care activities.
  • Ensure patient/family/significant others needs are met by coordinating and evaluating the use of resources and services in a quality-conscious, cost-effective manner.

Qualifications:

  • Education: Bachelor's degree in nursing or related discipline; Master's degree in nursing or related field preferred
  • Experience: Minimum of 3 years directly related experience in patient care management or related field
  • Bilingual in English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
  • Credentials: RN in NM or as allowed by reciprocal agreement by NM; CPR for healthcare/BLS provided or professional rescuers within 30 days

Physical Requirements:

  • Light work as defined with occasional lifting of up to 20 pounds and regular pushing and pulling of arm or leg controls.
  • Subject to hazardous materials that require the use of personal protective equipment and annual performance appraisals.
